Pharmaceuticals are another example of the preoccupation with “invention” over “enhancement.” Most people fail to appreciate the importance of adherence in determining the real world effectiveness of a drug. The harder a regimen is to adhere to, the lower the adherence rate and the less effective the drug. Even something as simple as introducing an extended release formulation (replacing a four times a day pill with a once a day pill) can dramatically increase adherence and can improve clinical outcomes by more than the invention of the pill itself. But economists tend to fixate on “new molecular entities” and ignore any subsequent tinkering.
